Congestive Heart Failure: Minimizing Risk with Early Warning Management

Take, for example, congestive heart failure, a long-term condition in which a person’s heart is unable to pump enough blood to the rest of the body. What might seem like regular symptoms of the condition, such as increased wheezing or fluid retention in the legs, otherwise known as edema, our nurses recognize as critical early warning signs. 

Our nurses help patients and caregivers understand what swelling or other changes may mean and when questions about diet, hydration, or medications should be discussed with the physician.

Proactive clinical support can also include medication oversight, such as recognizing when symptoms may warrant physician review of medications including diuretics such as Lasix. By coordinating with Primary Care Physicians to facilitate necessary dosage adjustments, we help reduce the likelihood of preventable hospitalizations. 

Beyond warning signs and medication management, we also provide a human touch to our delivery of education. We want your loved ones to feel in control through self-care steps they can take to manage their condition. While family members might have difficulty convincing a loved one to use compression socks, our nurses can act as a mediator in encouraging this behavior, ultimately taking the pressure off family.  

Dementia & Alzheimer’s: Care Beyond the Diagnosis

Chronic conditions involving memory loss are often difficult and complicated, requiring more oversight than a family member may be able to comfortably handle. Caring for these conditions is not limited to medication reminders; it can extend into how a home is set up and how daily conversations are held.

Outside of medication management, our nurses provide environmental safety checks of loved ones’ homes, such as conducting professional assessments for throw rugs, grab bars, and other household safety concerns so to help reduce fall risk and improve safety at home. Our nurses are also familiar with wheelchair transfer techniques and are willing to educate caregivers on them to allow loved ones to be mobile safely.
