Pre-op education
Post Surgical Care
Transportation to and from physician appointments
Coordination of care with physicians
Medication management
(education, reconciliation with physicians, prescription refill and pick up, pill box setup)
IV Antibiotics under a physician’s order
Catheter Care
Vital signs
Diabetes Management/Glucose monitoring, accu checks, insulin injections
Wellness check-ins at home, in a hospital, or care facility.
